Sign in to claim Lifes Choice 2

1. Enter your work email. We email you a sign-in link (no password).
2. The link brings you back to Lifes Choice 2’s claim page, where you tell us your role.
3. We call the facility’s listed phone number to confirm, usually within one business day.
